As with all similar items, the Belt Buckle is not used in the game beyond unlocking a journal page. The store description refers to the fact that Jethro Greensleeves was a ranch hand for Annie Stoakes who began committing crimes, eventually helping to burn down her ranch.
The Belt Buckle can be purchased from the Tailor in Brimstone after completion of the mission "Railroaded". If not purchased, it remains available during subsequent visits to Brimstone.
