Fern operates as the Barber in Smithfield's Saloon in Valentine, New Hanover.

Fern can be found in the Saloon, where he can cut and style the protagonist's hair. He can also be seen attempting to ignore Jon's drunken ramblings and insults.
Quentin Fern is also featured in the mission "Americans at Rest", where he can be seen among the crowd watching the brawl between Arthur and Tommy, cheering on the latter.

Fern has an aura of cordiality and will happily greet his customers accordingly. He generally keeps a calm and polite demeanor, barely showing any discontentment even with Jon's vulgar behavior towards him.

- His name is revealed in a conversation between him and Jon, during the random encounter, where the latter insults the barber, accusing him of being "a weed" and "whore-crazy", to which Fern cushions and asks him to stop.
- During the fight with Tommy in the mission "Americans at Rest", he can be heard cheering on Tommy by saying "Knock his head off!" and "Go on, Tommy!"
- After completing "The Spines of America", he will mention that Bob Crawford was robbed, when the player is getting a haircut.
- His loot will always consist of hair pomade.
